The rare concurrence of endometrioid carcinoma and cutaneous pilomatrix carcinoma, both showing mutations in the CTNNB1 gene related to beta-catenin, is a significant observation. Reports of high-grade tumors displaying this divergent differentiation pattern are exceedingly limited within the published literature. We document a 29-year-old woman's experience with an atypical presentation of endometrial cancer, the histology displaying features of a newly-characterized aggressive subtype, FIGO IVB grade 3 endometrioid carcinoma, with similarities to cutaneous pilomatrix carcinoma. A primary chemotherapy regimen initially yielded a substantial response in her treatment, but subsequent symptomatic brain metastasis necessitated whole-brain radiotherapy. Rarely do mesonephric neoplasms manifest in the lower female genital tract. Despite extensive searches, reports of benign biphasic vaginal mesonephric lesions are scarce, and none of the available reports have employed immunohistochemical and/or molecular analysis. A right salpingo-oophorectomy, intended for an ovarian cyst in a 55-year-old woman, led to the discovery of a biphasic neoplasm of mesonephric type within the vaginal submucosal area. Homogenous, firm, white-tan cut surfaces were evident in the well-defined 5mm nodule. Under microscopic observation, a lobular arrangement of glands was evident, featuring columnar to cuboidal epithelial cells and intraluminal eosinophilic secretions, situated within a myofibromatous stroma. Neither cytologic atypia nor mitotic activity was apparent. Immunohistochemical staining for PAX8 and GATA3 showed uniform expression in the glandular epithelium, while CD10 exhibited a variegated luminal staining pattern; no staining was detected for TTF1, ER, PR, p16, and NKX31.
